How it works

Clarity before commitment.

01

Request the service guide

Share basic contact information, your expected state during virtual care, and an optional area of interest. No medical history is requested on this page.

02

Meet your physician, then decide

A brief, no-cost, nonclinical introduction call is optional but encouraged. It gives you a chance to meet Dr. Deoli, ask administrative questions, and decide whether the practice feels right.

03

Begin through the secure system

If you choose to proceed and availability is confirmed, you complete intake and share detailed health information only in Oriva’s secure clinical system.

What you receive

A connected clinical picture—and a plan you can use.

Your history, prior results, goals, and physician assessment are brought together to identify what matters now and what should happen next.

Clinical synthesis

A physician interpretation of the important findings, what can wait, and what may require additional testing or referral.

Measurable targets

Individual expectations for relevant biomarkers, blood pressure, weight, waist circumference, body composition, and other meaningful measures.

Your daily plan

A detailed meal-by-meal nutrition framework with calorie and macronutrient targets; resistance and aerobic training based on your preferences and goals; and priorities for sleep, stress, and recovery, including a sleep-study recommendation when appropriate.

Results visit and roadmap

A dedicated physician visit to explain the findings and priorities, followed by a physician-signed Clinical Roadmap with clear next steps.

Recommendations depend on your history, findings, preferences, and clinical circumstances. Specific testing, referrals, treatments, medications, or outcomes cannot be promised in advance.

Who this may fit

For people who want the whole picture connected.

This may fit if you want…

  • A serious, evidence-based approach to long-term prevention
  • One physician connecting risk, lifestyle, body composition, and performance
  • Enough time to understand priorities, tradeoffs, and next steps
  • A written roadmap rather than a list of disconnected recommendations

This is not designed for…

  • Urgent or emergency concerns
  • Primary-care replacement or unlimited access
  • A prescription-on-demand service
  • Guaranteed testing, treatment, medication, or outcomes

Your physician

Dr. Igor Carvalho de Oliveira

MD, MPH · Board-certified in Preventive Medicine

Known publicly as Dr. Igor Deoli, Dr. Igor Carvalho de Oliveira focuses on preventive medicine, cardiometabolic health, cardiovascular risk, obesity medicine, body composition, lifestyle, and evidence-based performance care.

Oriva is intentionally structured around physician time: time to review information before the visit, conduct a comprehensive assessment, explain uncertainty honestly, and produce a practical roadmap. The practice maintains a deliberately limited patient panel to preserve that level of attention and responsiveness.

Common questions

What to know before requesting details.

Is the assessment a membership initiation fee?

No. It is a complete standalone physician service. The regular fee is $4,000; a $2,000 September preferred rate is available through September 30, 2026. No membership or recurring commitment is required.

What happens after the assessment?

You receive a physician-signed Clinical Roadmap and defined next steps. The assessment is complete once your results visit and written roadmap are delivered.

What costs are separate from the assessment?

Laboratory testing, imaging, devices, facility fees, and services from outside clinicians are arranged through third parties and paid separately. Oriva does not add a markup. Expected outside costs and available options are discussed before you proceed.

Does this assessment include treatment or prescribing?

No. This is a distinct non-prescribing assessment. It provides clinical synthesis, individualized goals, recommendations, and a written roadmap. It does not initiate or adjust medication, provide ongoing treatment, or function as a prescription-access service.

Are GLP-1 medications available through the initial assessment?

No. The initial assessment does not include a GLP-1 prescription or medication management. If you are eligible, are accepted into the separate longitudinal-care program, and choose to continue, clinically appropriate prescribing and monthly follow-up may be included in the professional service. Medication costs are separate.

Is hormone replacement therapy available through the initial assessment?

No. The initial assessment may evaluate hormonal health and recommend appropriate next steps, but it does not initiate hormone therapy. If you are eligible and choose separate longitudinal care, clinically appropriate prescribing and monitoring may be included in the professional service. Medication and outside testing costs are separate.

Does the initial assessment include ongoing management of type 2 diabetes or hypertension?

No. The standalone assessment can evaluate risk, establish individualized goals, and provide a roadmap, but it does not provide ongoing disease management. Eligible patients accepted into the separate $1,000-per-month longitudinal-care program may receive ongoing management, clinically appropriate prescriptions, and monthly follow-up. Medications, laboratory testing, imaging, devices, facilities, and outside clinicians are billed separately.

Do I have to book an introduction call?

No. The call is optional, but encouraged. It is a brief, no-cost opportunity to meet Dr. Deoli, ask administrative questions, and decide whether the relationship feels right before beginning the clinical process.

What should I put in the form?

Only basic contact and administrative information. Do not include symptoms, diagnoses, medications, laboratory results, medical records, or urgent concerns.
